Blizzard didn’t really want to make a red-eyed monster.
Lilith, the main antagonist of Diablo 4, is heterochromic – her eyes are of different colors. This is rare for people, but it happens, but Lilith is not a person.
As junior development lead Joe Piepiora explains, giving Lilith multi-colored eyes had several goals in mind for Diablo 4’s designers. Thus, the developers show the essence of Lilith and make it clear that she is not quite a monster.

Even among demons, Lilith is considered an outcast. She has eyes of different colors: one is blue, the other is grey. The blue eye represents her good side, yet she’s not exactly a villain. We specifically didn’t want to make her another red-eyed demon.
In the lore of Diablo, Lilith hasn’t really always been a villain. Together with the angel Inarius, Lilith is considered the creator of the Nephalem, the distant ancestors of people. However, over time, the relationship between Lilith and Inaria deteriorated and now they are fighting each other.
Diablo 4 will be released on June 6, 2023 on PC and consoles of two generations.
