In the past two days, players noticedon the Xbox console, they achieved the “Ghost” achievement in some old projects. Games that were previously 100% complete start showing as unfinished. Despite getting 1000 points out of 1000 points (or more) in the game, the profile shows that the project has a completion rate of less than 100% and the total number of achievements has increased.
Xbox Chief Technology Officer Scott Van Vliet explained that there is a glitch related to challenges used in some Xbox One projects. Many of these are temporary, tied to a specific period or event, and are no longer available. Technically, however, the Xbox system counts them as achievements with a zero player score. In the context of server logic changes, such challenges began to be taken into account when calculating the total number of achievements.
For example, if a game has 50 regular achievements and some old challenges, the system may temporarily display 52 achievements. Because of this, the progress is less than 100% and it is impossible to obtain the “missing achievement”.

Due to an unexpected change in the service, legacy challenges were included in the overall achievement count for some older games, which also affected the calculation of the displayed completion percentage (fun fact/explanation: behind the scenes, challenges were counted as achievements with a player score of zero). This is a display issue and underlying player data is not affected.
We’ve fixed the way completion percentages are displayed, and the achievement totals displayed will be corrected over the next few days. You don’t need to update the client to do this. Thank you for your patience!
Note that players also discovered that Xbox plans to add the “Mythic” achievement, which will likely become a platinum achievement similar to the Playstation one.
